Looking for a dynamic Case Manager who will coordinate the overall disciplinary plan of care for patients, from admission to discharge.
This would be Saturday and Sunday only.
Qualifications Education:
Minimum of a Bachelor's Degree in related field plus 1 year of experience in a healthcare related position.
Case Manager roles and responsibilities include coordinating, advocating, linking and monitoring services to help individuals access needed medical healthcare, behavioral care, basic needs, housing, educational, social and other services.
The Case Manager:
Coordinates the overall interdisciplinary plan of care for patients, from admission to discharge.
Monitors care and acts as a liaison between patient/family, healthcare personnel and insurers.
Evaluates the needs of the patient, resources available, and recommends and facilitates the plan for the best outcome.
Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice.
Identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care.
May be responsible for conducting utilization reviews.
