Pay begins $34/hour and can go up depending on experience.
We offer benefits, mileage reimbursement (around $200 a week for FT clinicians!), and minimal on- call requirement (We have a team for that!) Currently, we are looking for a Registered Nurse Case Manager who would like to combine their love for people and excellent clinical and leadership skills with the opportunity to advance your career.
The Position Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous illness (es).
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs and participating in revising the plan of care as necessary Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Supervises ancillary personnel and delegates responsibilities when required.
Qualifications Graduate of an accredited school of nursing.
1 to 2 years of recent acute care experience and 1-2 years of recent experience in home care Good standing RN license in state of practice BLS Hands-On CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
